This service does not replace GP visits or an ambulance in an emergency.
What is Healthy@Home?
Healthy@Home is a free phone support service, to help people stay well and at home.
Who can access this service?
The Healthy@Home service is available for anyone aged 18 years and over, who may be at risk of a hospital admission.
What do we do?
Healthy@Home telehealth assistants will call you regularly to see if you have any concerns. You can choose how often we call and you can cancel these calls at any time.
The phone call will cover questions about
- Your health
- Medication and medical care
- Social support
- Your environment
- Your care
What happens if help is needed?
Healthy@Home telehealth assistants ask questions that can identify if your health is declining. They will connect you with the services that you need. A trained health professional will call or visit you to organise the appropriate support.
