Where is our District Nursing available?
Our District Nursing service is available to those living in Beaufort and Skipton areas – the service is available to people living within approximately a 35km radius of Beaufort and Skipton.
What do we do?
- Our District Nursing team aim to support and assist you to maintain your health and retain independence in your own home.
- Care is undertaken by Registered Nurses and Medication Endorsed Enrolled Nurses who can provide an assessment of all your health needs. They will work with you and your family or carer to develop a personalised care plan to suit your needs.
- We provide services including wound management, medication management, nursing support, palliative care nursing, care following hospital discharge, assessment and referral to other service providers.
Who can access this service?
District Nursing service is available to any community member who needs nursing care in their home – subject to assessment by one of our District Nurses.
Do I need a referral?
Referrals can be made by any authorised health care provider. In some circumstances self-referral or family referral may occur.
For Department of Veteran’s Affairs (DVA) clients, a doctor’s written authority is required to access the service.
Cost
Pensioners or health benefit cardholders pay an agreed amount that will be discussed during the first visit.
Other fees are charged according to client income.
Please note our staff require a smoke free workplace, we request no smoking while we visit. Prior to starting visits and care, staff complete a client and home assessment. Occasionally changes need to be negotiated, to reduce risk, before staff can visit.
