Our Diabetes Education service is provided by a Credentialed Diabetes Educator who can support you to self-manage diabetes.
Where is our Diabetes Education available?
Our Diabetes Education service is available at both our Beaufort and Skipton campuses.
What do we do?
- Help you manage your diabetes
- Help you understand what diabetes is
- Teach blood glucose monitoring, understand what your results mean and what can affect the results
- Provide information and explain medications and insulin
What should I bring to my appointment?
List of current medications, blood glucose record book, recent blood test results, blood glucose meter and equipment.
Do I need a referral?
No, you don’t need a referral to see our Diabetes Educator.
Cost
$10. There will be no cost if you have a referral from your GP as part of your GP Management Plan and Team Care Arrangement.
