Transition Care Package Coordinator

Our services are critical to our community’s needs.  We are seeking a highly motivated and passionate individual to coordinate our TCP program.  You will provide assessment, support, care planning and case management to eligible clients and their carers in a bed-based program or an in-home based service.  You will coordinate the timely management of clients as they complete their restorative process, optimise their functional capacity and access their long-term care arrangements.
