Person Centred Care
At Beaufort and Skipton Health Service we respect the patient/resident experience. This is demonstrated in our commitment to delivering person centred care.
In the broadest terms, person centred care is care organised around the aged care residents or patients engaged in the services that Beaufort and Skipton Health Service offers. It is a model in which we partner with residents, patients, families or carers to identify and satisfy the full range of resident or patient needs and preferences.
The core concepts that we believe is at the heart of person centred care includes:
- All nursing, personal carers, medical officers and other health care practitioners listen to and honour resident, patient and family/carers perspectives and choices. Resident, patient and family/carer knowledge, values, beliefs and cultural backgrounds are incorporated into the planning and delivery of care.
- Health care practitioners communicate and share complete unbiased information with patients and families in ways that are affirming and useful. Residents, patients and families/carers receive timely, complete and accurate information in order to effectively participate in care and decision making.
- Residents, patients, family/ carers are encouraged and supported in decision making at the level they choose.
- Residents, families/ carers are also included in decision making across the health service.
