Home Care Package Care Coordinator

We have an exciting opportunity for a suitably qualified and experienced Care Coordinator to join our team. This role is 0.6 (3 days per week) and the rate will be commensurate with experience.
You will be responsible for the coordination of care packages to assist clients to remain living independently in their own homes with the appropriate support services.
You will need to be highly organised, with attention to detail and a genuine interest in the client’s wellbeing. You will need to perform will within a close-knit team environment but also be able to operate with high levels of autonomy.
